Remote Patient Monitoring (RPM)

Those patients with Medicare and at least 1 chronic condition are eligible for Remote Patient Monitoring.

Remote Monitoring can also be effective in surveillance for suspicion of illness including COVID19.

Remote Patient Monitoring (RPM) is an option for your Provider to monitor your status remotely through devices (which do not require any app or connectivity) that utilize cell phone frequencies to communicate to a central computer system.

After consenting (verbally or in writing) you will be enrolled in the program, sent devices and setup for training.

The devices are sent fully configured and ready to use, additionally disposables (batteries, etc.) covered by Medicare.  Data is streamed in real-time through cellular services from the patient’s device to your provider.  In the event a patient wishes to disenroll, the devices do not need to be returned and the RPM program will discontinue.

Advanced Devices:

  • Blood Pressure Cuff

  • Blood Glucose Monitor

  • Digital Weight Scale

  • Emergency Watch
